GENERAL OVERVIEW:
The Registered Nurse is a professional nurse registered in Massachusetts who is responsible and accountable for planning and providing patient care for assigned patients in accordance with Nursing Standards of Care as set forth in the Nursing Department’s Clinical Practice Manual.

The registered nurse demonstrates initiative, knowledge and clinical skills in caring for the

patient with complex needs. The Registered Nurse demonstrates the ability to effectively

manage patients by assuming full responsibility for the assessment, plan, implementation and

evaluation of patient care and is directly responsible to a designated nurse manager, or

supervisor.

PRINCIPAL DUTIES AND RESPONSIBILITIES:
CLINICAL PRACTICE:
A. Organization of Patient Care:
1. Accountable for assessing, planning, implementing and evaluating a plan of care

for a specific patient assignment.

2. Sets priorities when organizing care for patients with varying acuity.

3. Maintain continuity through clear and concise (verbal and written) communication.

4. Demonstrates appropriate knowledge of growth and

development of the adult and geriatric patient. In areas, such as the NICU and Obstetrics

the principals of growth and development are applicable to the neonate.

B. Quality of Care:
1. Documents patient care in a manner that is clear, complete, concise and in compliance with

nursing documentation standards.

2. Develops a comprehensive plan of care based on data from an initial assessment of patient

and family, information from other members of the health care team, intra-agency referral

and previous medical records.

3. Develops a comprehensive educational plan for the patient and family, utilizing appropriate

resources and documents according to the department’s documentation standards.

4. Develops a comprehensive discharge plan utilizing appropriate resources and referrals

including community resources.

5. Evaluates the effectiveness of the plan of care, and documents progress in meeting

stated goals. Revises plan of care as needed to achieve desired outcomes.

6. Participates actively in the unit-based and organizational quality management and/or

quality improvement programs.

7. Provides a safe environment for patients, staff, family and visitors.

8. Administers medication safely according to established policies and procedures.

9. Performs nursing procedures safely and efficiently.

10. Uses equipment safely and efficiently.

11. Demonstrates awareness of potential/actual risks of infection and modes of transmission.

Utilizes universal precautions in nursing practice.

C. Coordination/Collaboration:
1. Interacts with patients, families, and colleagues in a professional manner.

2. Collaborates with other discipline(s).

3. Develops, utilizes and evaluates unit-specific standards of care.

4. Upholds the A.N.A. code of ethics and acts as a role model to other staff members.

Requirements:
QUALIFICATIONS:
A. Education:
Graduate of an approved school of nursing with current registration in

Massachusetts. For newly licensed nurses a Bachelor of Science Degree inNursing is required.

B. Experience:
Prior experience not required unless specified for certain specialty areas.

C. Other:
Membership in professional organizations is recommended and certification in

a specialty area is encouraged.

SKILLS AND ABILITIES REQUIRED:
The RN must show evidence of the basic analytic thinking necessary to care for a group of

patients. Must demonstrate observational skills and the ability to set priorities. Must be able to

function under stress with good interpersonal and communication skills. Must demonstrate

effective skills in applying hospital standards in area of service, team work, communication,

respect for others, and time/priority management.

WORKING CONDITIONS:
Works in a variety of patient care environments where there may be exposure to communicable

GENERAL SUMMARY/ OVERVIEW STATEMENT:
The Cardiac Catheterization RN is a professional nurse registered in Massachusetts. This position requires sound judgment and advanced nursing skills to provide patient care in a wide variety of cardiovascular and interventional procedures. The Cardiac Catheterization RN acts as a critical member of the patient care team within a fast paced, high acuity area that requires independent thinking and critical judgment skills and flexibility within an ever changing environment. The Cardiac Catheterization RN collaborates fully with other care team members in assuring patient and family centered care. The Cardiac Catheterization RN promotes a philosophy of patient / family- focused care in all interactions with patients, families and hospital personnel and focuses on understanding and meeting customer needs. He/she will provide hospital / unit information for all customers upon request.

PRINCIPAL DUTIES AND RESPONSIBILITIES:
Collaborates with all team members of the CDIC with planning an implementation of patient care and assures continuity of care. Completes conscious sedation pre, procedural and post assessments and practices under standard guidelines developed in conjunction with the anesthesia department, JCAHO regulations, and Massachusetts RN licensure guidelines.

Completes nursing assessment prior to procedures. Assists in preparation of the patient in the procedure room. Provides one to one monitoring during the cardiac cath lab procedures adhering to Cardiovascular Diagnostic and Interventional Center (CDIC) nursing standards. Responsible for continuous assessment of patient condition, needs, and responses to therapeutic measures. Functions as a team member in cross-trained activities. Provides complete documentation of patient status and communications needed to assure continuity of care. Collaborates with interdisciplinary services (i.e. pharmacy) to develop optimal plans of care.

Assures continuum of care and education throughout the cardiac catheterization experience. Provides preadmission screening, education and work-ups for cardiac catheterization patients and families as directed. Supervises ancillary workers and performs or assists in sheath care. Maintains care plans and provides teaching of care for interventional cardiology patients until discharged from CDIC services. Evaluates provision of care and effectiveness of teaching material, education and provides discharge planning and teaching collaboration with team members and referring agencies. Provides follow up calls to patients post discharge as directed.

Demonstrates ability to effectively handle emergency situations. Maintains BLS, ACLS, and IABP competencies and critical care skills. Develops and maintains competencies in IV therapy and phlebotomy. Maintains working knowledge of active research protocols pertaining Cardiac Cath Lab patients, along with the corresponding medication and equipment. Performs Point of Care Testing (ie, testing of blood glucose, co-oximetry and Activated Clotting Time) according to BWH Lab Control and JCAHO Policy.

Maintains high level of competency in cross-training activities, and ensures regular rotation through all caseload types in conjunction with the NIC and Nurse Manager.

Adheres to universal precautions, infection control and safety standards of CDIC and acts as a resource in maintaining infection control practices within the team. Adheres to Brigham and CDIC standards of nursing practice.

Assists Flow Coordinator, NIC and team members to facilitate patient flow and room turnover through CDIC. Assists in the transportation of patients requiring monitoring and as needed. Promotes an interdisciplinary approach in providing patient sheath care with colleagues on the nursing units.

Participates in appropriate and professional programs with supports and improve expertise in CDIC services. Participates in educational programs with CDIC. Participates in staff meeting and in-services. Assumes responsibility for on-call coverage as assigned.

Assists in developing and implementing policies and procedures related to patient care management. Assists in the completion of quality assurance and improvement programs under the direction of the administrator and nursing quality specialist.

Performs all other duties as assigned.

Performs ACT moderately complex testing and interpretation according to established guidelines.

QUALIFICATIONS:
Graduate of accredited school of nursing; BSN strongly preferred.

Current MA RN license, prior to practice.

2-3 years of prior Critical Care experience required. Prior Cardiology and/or Cath Lab experienced preferred.

On call required with a 30 minute response time

SKILLS/ ABILITIES/ COMPETENCIES REQUIRED:
Must possess strong interpersonal and communication skills to interact effectively with patients and promote interdisciplinary collaborative teamwork, including in emergent situations. Maintains effective working relations and communication with departmental and other hospital personnel.

Must possess strong analytical and organizational skills necessary to acquire and effectively utilize knowledge of critical care nursing practice integrated with CDIC procedures.

Must demonstrate interest and initiative in learning cross-trained responsibilities defined within the department.

Work requires extensive orientation and precepting to acquire necessary skills required to perform conscious sedation and provide nursing care in preparation areas, multiple types of procedures and patient discharge.

Must be able to prioritize multiple responsibilities encompassed within the position. Must have the ability to adapt to changing pace within the department.

Must be able to promote a customer-oriented philosophy among all staff within the area.

Uses independent judgment, creativity and initiative in a variety of situations when performing patient assessment and care.

Ability to work under extremely stressful conditions.

Must be able to understand and operate monitors, emergency equipment and multiple computer information systems, including but not limited to computer charting of patient condition and computerized scheduling of patients.

Must be physically capable of lifting (linen, supplies, patients, etc.).

Must be aware of hazards within the department; adhere to safety standards established by the hospital and other regulatory agencies.

WORKING CONDITIONS:
Work requires a considerable amount of walking and standing while wearing heavy lead.

Requires physical effort to lift and position patients and radiographic equipment.

Exposure to some radiation during fluoroscopy, spot filming and digital acquisition in labs.

Exposure to blood and waste products and contagious diseases.

Constant changes in lighting â€" overhead lighting, followed by dark during flouroscopy, bright lights on illuminators and surgical lamps, dark in the reading room etc.

Stressful high-volume, high acuity patient care environment.

Requires call responsibility with an on call response time of 30 minutes.
