Description: LYNN COMMUNITY HEALTH CTR is looking of RN Case Manager Ob/Gyn right now, this position will be placed in Massachusetts. RN Case Manager-Ob/Gyn Provide assessment, care planning, education, follow up and support of patients in community health center. Collaborate with health center providers and multidisciplinary professionals in providing for physical, psychological and/or social support for assigned patients/families, identifies and establishes relationships with community resources. RN License and experience in Ob/Gyn, Nurse Case Management required, bilingual a plus. No nights or weekends!
