The practitioner has responsibility for assessment and management of patients including diagnostic and therapeutic interventions, and continuous implementation and evaluation of an appropriate plan of care. The Nurse Practitioner works under the supervision of the attending physician and is responsible to the Nurse Manager, or Director for the area PRINCIPAL DUTIES AND RESPONSIBILITIES:
Patient Care Organization
Provides direct care, counseling, and teaching to a select patient population.
Performs physical examinations and provides preventive health measures appropriate to patient needs.
Orders, interprets, and evaluates appropriate lab and diagnostic test to assess patient’s clinical problems and healt! h care needs.
Arranges for appropriate plan of care and fo! llow-up based on outcome of diagnostic, lab, and physical assessment findings.
Performs laboratory tests appropriate for specific practice area.
Orders medications and writes prescriptions according to organizational and regulatory policies and procedures.
Records pertinent patient care findings in the medical records.
Set priorities for appropriate and efficient management of patient care that reflects cost effective management of time, available human resources, supplies and equipment.
Maintains interdisciplinary approach to planning and communicating patient care information by discussing patient data with appropriate physicians and other health care professionals.
Incorporates the concepts of health maintenance, prevention, and promotion into daily practice through patient education and counseling.
Demonstrates awareness of community resources and assists staff, patients, and families to effectively utilize them.

Quality of Care
! Assist in the development, implementation and revision of nurse practitioner guidelines.
Evaluates outcomes of own patients’ care and makes necessary revisions in care plans.

Planning
Develops annual objectives and reviews with Nurse Manager and/or Director at time of performance review and at appropriate intervals.
Assists in determining, evaluating, and/or implementing clinical area goals and objectives.
Maintains interdisciplinary approach for planning and communicating information for patient care and operation of practice.
Maintains current knowledge of legislation that may affect the health care delivery system.
Research
Participates in nursing researc! h at a level consistent with educational background and experience.
Maintains understanding of current research of others in related clinical area through review of relevant publications.

Requirements: QUALIFICATIONS: Education : Master’s degree in Nursing required. Satisfactory completion of an approved formal education program which has as its objective the preparation of professional nurses to practice as nurse practitioners. Experience : Experience with diabetes is preferred. Preference also will be given to individuals with a commitment to becoming a Certified Diabetes Educator as soon as they are eligible. Other : ANA or other appropriate nursing certification in the area of practice and authorization by the Massachusetts Board of Registration in Nursing to practice in an expanded role. DEA for prescription writing. SKILL/ABILITIES/COMPETENCIES REQUIRED:

EEO Statement: An EEO, AA, VEVRAA Employer Additional Information DESCRIPTION OF THE PRACTICE: The nurse practitioner works in conjunction with members of the! Brigham and Women’s Hospital Department ofEndocrinology, Diabetes, and Hypertension and a liaison with the Associate Chief Nurse of Medicine. Patients seen by this practice are adults presenting with Type 1 or 2 diabetes, stress hyperglycemia, or gestational hyperglycemia. Care provided within this setting is defined asinpatient consultation and follow up, outpatient diabetes management, and outpatient diabetes education.Patient care is provided seven days a week in multiple locations, which requires some local travel and weekend coverage. NATURE AND SCOPE OF PRACTICE: The nurse practitioner practices and facilitates the implementation of evidence based care standards for diabetes and hyperglycemia for adult patients on the inpatient Diabetes Management Service, in the outpatient Diabetes Clinic, and in the Diabetes in Pregnancy Program. The nature and scope of nurse practitioner practice includes patient and family assessment, diagnosis, development of a plan of care, t! reatment and evaluation of treatment and education. The nurse practitio! ner works in collaboration with clinical staff (attending, fellow, house officer, physician’s assistant, clinical nurses, nurse practitioners, nutritionists, pharmacists) from outpatient diabetes management/education in the diabetes clinics and inpatient surgical services, including Obstetrics, Women’s Health Center at 850 Boylston Street, Faulkner Hospital, Patriot Place and any other BWH service requesting consultation from the Diabetes Management Service. Specific responsibilities include, but are not limited to:
Complete patient evaluation including performing and documenting a complete history and physical examination
Ordering and interpreting diagnostic test and regimens
Recommending and ordering blood and diagnostic tests
Interpreting blood tests
Recommending consultations
Recommending, ordering, and evaluating medications and therapeutic regimens
Symptom management
Pre and post operative evaluations
Recommending, ordering and ! evaluating medications and therapeutic regimens
Teaching and assisting patients in the use of technology, including insulin pumps and CGMS as advisable.
Consulting with the patient, patient’s family, and interdisciplinary team to determine the plan of care.
Discharge planning.
Assessing the needs of a patient’s family and caregivers.
Participating fully in performance improvement, clinical research, accreditation, compliance, and regulatory activities.
Implementing and developing evidence-based guidelines, in concert with the Directors of Inpatient and Outpatient Diabetes, attending MDs, and trainees.
Additional professional activities including acting as preceptor for student NPs in the role of midlevel practitioners in this environment.
Developing and improving Diabetes documentation.
Participating in professional organizations (ADA, AADE, AACE) that foster interaction and shared learning with other Diabetes practitioners at other i! nstitutions.
Provide comprehensive diabetes education to adults wit! h type 1, type 2, and gestational diabetes.
Participate in educational programs for health care professionals including MDs, nurses, NPs, nursing and medical students, dieticians, pharmacists, and other allied support staff. DEPARTMENT SPECIFIC QUALIFICATIONS: CLINICAL STANDARDS THAT SERVE AS GUIDELINES FOR THIS PRACTICE Nurse practitioner practice conforms to generally accepted standards of care as promulgated by the American Nurses Association and other relevant professional organizations.
