Under the supervision of the Director of Quality Assurance and Improvement, the Summit Community Care Clinic (SCCC) Population Health Specialist provides agency-wide support for managers and teams to meet and exceed the organization’s quality standards and expectations. The Population Health Specialist will assist in identifying common patterns in the health-specific populations and developing and implementing plans to improve health outcomes and the well-being of those populations.

The Population Health Specialist is responsible for patient outreach and support for various quality incentive measures and assists with data quality assurance tasks. This is an onsite position and will need to be available to travel to our various locations, as needed.


As a condition of employment, Summit Community Care Clinic requires COVID-19 vaccination of all clinical and non-clinical staff.


Essential Functions:

  • Perform patient outreach for Care Clinic quality incentive measures via mailers, phone calls, text, email campaigns, community events, groups, or other outreach methods needed to establish contact and build effective connections with the patient population.
  • Perform regular audits of patient attribution lists and care gaps and execute needed outreach to patients to fulfill requirements.
  • Through outreach activities, help identify women due for breast and cervical cancer screening. Work collaboratively with the eligibility department and medical team in helping identified women get scheduled and complete cancer screening services.
  • Assist with patient notification letters and patient-provider reassignments as needed.
  • Assist in identifying patient barriers and identification of supportive services and resources available to patients to meet specific needs.
  • Assist patients to access and navigate medical, behavioral health, dental, and other specialty care, as needed.
  • Assist patients to connect with supportive services and resources both within SCCC and throughout the community that is critical to their overall health and well-being.
  • Assist with select Quality Improvement/Quality Assurance or other projects as directed to support teams in achieving goals and objectives.
  • Assist the Director of Quality Assurance and Improvement with data collection and validation.
  • Assist the Director of Quality Assurance and Improvement with projects related to data integrity.
  • Other duties as assigned.

Additional Requirements:

  • Grants are a vital part of Care Clinic revenues and directly or indirectly support all positions. This role may have responsibilities that are closely associated with grant deliverables. Grants and grant responsibilities may change over time.
  • Complete all required training as designated by HR and the position supervisor.

  • Comply with all SCCC Vaccine Mandates.
  • Willingness to travel and work at other SCCC locations as determined by the supervisor.
  • Comply with all applicable Credentialing and Privileging requirements timely.
  • Embrace the SCCC Mission and Vision and agree to work within the SCCC Guiding Principles and the principles set by the position supervisor.
  • Must participate in quality assurance/ improvement activities when applicable.
