More details about this job opportunity kindly see the descriptions. PART TIME CASE MANAGER for Boston Healthy Start Initiative grant funded position within Ob/Gyn Clinic. Responsible for providing intensive case management services for Black women who are in their first trimester of pregnancy and followed 24 months after delivery. Comprehensive services provided include outreach, health education, family planning, screening patients for depression, linking clients to primary and pediatric care, identifying and connecting patients with health and social services and patient advocacy. Two years experience providing direct service to pregnant and parenting women, preferably in community-based setting, culturally and linguistically competent, Associates degree in human services, nursing or related field.
