Description: Uphams Corner Health Center is employing Adolescent/Family Social Worker right now, this job will be placed in Massachusetts. For detail informations about this job opportunity please give attention to these descriptions. The position is shared between the Teen Clinic and the Social Services Department. The Adolescent Health/Generalist Social Worker coordinates the Social Service! needs of adolescent patients in the Teen Clinic by providing counseling and concrete services, as well as outreach into the community. The position is responsible for delivery of Social Services to the Primary Care, Urgent Care and OB/GYN Departments.
Collaborates with the adolescent health team to identify, offer outreach and deliver care for those patients in need of psychosocial services.Provide links for adolescents to community resources in the areas of employment, education, housing, substance abuse services, etc.Maintain and complete, in a timely manner, all appropriate paperwork and documentation in compliance with Social Services Dept. policy and procedures.Develop and facilitate psycho-educational social support groups at both the Teen clinic and Social Services Dept. site.Develop and build upon existing collaborative relationship with external agencies related to Adolescent social services needs.
